I am over 18 years of age and can provide a valid driver's license or ID card.

I am not pregnant or nursing.

I do not have a history of cold sores at the proposed procedure site, and if I did, I have informed my technician to be able to prepare for the service. 

 I do not have skin irritations, Eczema or Psoriasis on the treated area (rashes, sunburn, acne, etc.)

I have not had Accutane in the past 12 months. 

I have not had Botox, Micro-needling, Facials, Laser treatment on/around the area in the past month. 

I do not have epilepsy, diabetes, allergic reactions to latex or antibiotics, hemophelia or other bleeding disorders.

I do not have cardiac valve disease or suffer from any major heart conditions or take medications that thins my blood.

I have advised my technician of any and all medications that I am currently taking. 

If I suffer from hepatitis, or any other communicable disease, or have any risk for possible bloodborne pathogen exposure, I have informed the technician prior to any service. 

If I have had previous work done, I have already sent a photo for approval before booking.

I do not have any keloids, and am not prone to keloid scarring
